The Reset Challenge Terms & Conditions

1. Participants must obtain consent from a parent or guardian before joining the programme.

2. Participants must attend their first consultation with a parent or guardian and recommend that a parent or guardian joins all future appointments where possible. If this is not achievable, please discuss this with your Advisor.

3. Participants are required to attend all scheduled weekly consultations face to face with their Be Smoke Free Advisor. Remote or telephone appointments are not available for this challenge.

4. Maintain ongoing communication with your Advisor.

5.Complete all necessary documentation, including carbon monoxide tests and lifestyle questionnaires, throughout the duration of the programme.

6. You must notify your Advisor of any health changes that may influence your ability to attend sessions or impact your wellbeing when using prescribed stop smoking (Nicotine Replacement Therapy).

7. Participants must always conduct themselves in a safe and appropriate manner.

8. By registering for the Reset Challenge and adhering to these terms and conditions, you will qualify for the following incentives:

• A complimentary 12-week Everybody Health and Leisure membership upon registration, including swimming, classes and gym. Refer to point 12 for further information in gym related activity.
• A £20 Love 2 Shop voucher after a successful 4-week quit.
• An additional three months of free membership upon completing the full 12-week support programme.

9. Your membership will not be frozen or suspended during the 12-week programme unless you have exceptional circumstances.

10. Activities included in the membership are strictly for the named member only.

11. You must complete an ‘Activation’ appointment prior to using the gym and a parent or guardian must sign a Health Commitment Statement beforehand. Booking your Activation appointment through site reception.

12. Young people aged 12-13yrs may use the gym only by attending Next Gen Gym sessions; from 14yrs they may access the gym independently.

13. You must inform us of any changes to your health and medical history, that could affect your ability to exercise safely.

14. Failure to comply with these Terms and Conditions will result in removal from the programme and forfeiture of any remaining incentives.

15. Completion of this scheme makes you ineligible for future re-enrolment in the Reset Challenge.

16. Accepts no liability for death, personal injury or any liability for death, personal injury, or form of loss or damage.

17. Reserves the right to change, amend, or remove any of the above conditions or activities, or to suspend the Reset Challenge at its discretion.

18. Your personal data is managed in accordance with the General Data Protection Regulation (EU) 2016/679 (to process your membership and share your data with Cheshire East Council commissioner of the service).
